9to5 Georgia is collecting information to assess the state of childcare in Georgia. This information will be used for research purposes only and will remain confidential. Any homes operating without a license while caring for more than 2 unrelated children for a fee will not be reported.

9to5 Georgia is interested in learning more about Family, Friend, and Neighbor (FFN) care providers and how to support them. FFN care providers care for children for a relative, friend, or neighbor for pay or for free. A relative or family member can be someone connected to you by blood or marriage. FFN care providers are unlicensed and care for the children in their own homes. If you are a FFN care provider, please complete the following survey.

Participants who complete this survey will earn a $10 eGiftcard to their choice of Target or Walmart, and will be entered into a drawing to win a $50 gift card at the end of the submission period as well. You must provide your name, address, and email address in order to be eligible to receive either gift card incentive.
